ADAPT Community Network Programs & Services

Project Connect
Sorting through information and securing opportunities for a loved one with a disability can be an arduous and challenging process. Project Connect, with trained and understanding staff to expertly guide anyone who calls to the services and offerings they need, is the place to start.Project Connect staff are available five days a week to respond to inquiries and offer assistance and support. If we are unable to provide answers to inquiries or direct individuals to services within ADAPT Community Network, we will offer referrals to other agencies and organizations.

Technology
Assistive Technology finds solutions to enable individuals to adapt to their environment with a wide range of options. ADAPT Community Network provides evaluation, selection, instruction, and introduction customized to each individual’s needs. Our Assistive Technology includes vision, hearing, communication, daily living, and environmental control modifications.

Education Services
We support children of all abilities, working together to prepare them for life. ADAPT’s commitment to Children’s Services includes comprehensive education for children and young adults. Our preschool, school age, and after school programs are specifically designed to engage and educate children with special needs. Our universal pre-kindergarten and integrated programs offer families a high-quality family friendly learning environment for children of all developmental levels. All classes are led by licensed and certified teachers and are staffed with certified teaching assistants. We also offer after school programs, a summer day camp and educational resources libraries.

Employment Services
At ADAPT Community Network, we are committed to finding employment opportunities tailored to the individual’s interests, abilities and strengths. We work to match job seekers with the right job based on their strengths and interests. We maintain an ongoing presence in the individual’s professional life, offering vocational training, job development, job site adjustment, professional development, job coaching, adaptive technology, and ongoing counseling and support as needed.

Healthcare
Adapt Community Network offer a broad spectrum of health care services to individuals with disabilities and their families. All ADAPT Community Network clinics are accessible, offering medical and dental primary care, preventative planning, as well as specialty clinical and therapeutic services. Our wellness philosophy supports the active participation of the individual in every phase of the health care process.

Living
ADAPT Community Network sponsors a variety of living options for persons with intellectual and developmental disabilities designed to meet the needs of each person.

Family Reimbursement
This provides critical financial support to eligible family members for everything from uninsured medical supplies to a computer, summer camp fees to adaptive equipment.

Housing Assistance
The Housing Assistance Program provides a variety of services to eligible individuals and their families throughout New York City. Services include housing and accessibility rights education, landlord/tenant negotiations, code enforcement advocacy, help in expediting public housing applications, transfers, referral to legal services, evaluations for adaptive equipment and home modifications.

Doorways To Independence
Simple home alterations can be life changing, helping individuals adapt to their home environment.

RESPITE
Caregivers sometimes need a break. We can help with that. We offer the only freestanding, wheelchair accessible overnight respite home in New York City, located in a park-like setting in the Bronx. We offer 24-hour supervision with nursing services to individuals of all ages with developmental disabilities.

Community Habilitation
The Community Habilitation program at ADAPT is a person-driven, outcome-based service tailored to individual needs, wants, and preferences through the implementation of valued outcomes/goals. We provide 1 staff to 1 participant for those living with families or on their own in Brooklyn, Manhattan, Staten Island and Bronx.

Family Connect
ADAPT’s Family Connect offer free workshops, support services and educational resources for New York City parents and families of children with disabilities. Families are encouraged to learn, explore resources and participate in improving outcomes and supports for all children with disabilities.

Early Childhood Education Center
The Brooklyn Early Childhood Direction Center (ECDC) provides information and referral services to families and young children who have suspected or diagnosed developmental delays or disabilities. Children referred must reside in Brooklyn and be five years of age or younger.

Action Day Camp
Action Camp includes a variety of recreational and socialization activities such as swimming in our heated indoor pools and use of our adaptive gym, and many more adventurous and exploratory outings. This takes place during the late summer gap between the end of most summer school programs until the start of the school year (mid-August). Action Camp is for eligible Brooklyn resident teenagers.

Spirit Recreation
ADAPT recreation includes outings and leisure activities including trips to zoos and museums, dining out, cooking and baking, movies and theatre, and a wide range of social activities.

Arts Programs
The robust arts program at ADAPT includes poetry, painting, media arts, and other opportunities for self-expression. Children and adults can take elective classes and work on independent projects. Individual’s art is showcased in galleries and art shows, displayed on the walls of businesses and within our own programs.

Spinal Cord Injury

Spinal cord injury can result in paralysis of the muscles used for breathing; paralysis and/or loss of feeling in all or some of the trunk, arms, and legs; weakness; numbness; loss of bowel and bladder control; and numerous secondary conditions including respiratory problems, pressure sores, and sometimes fatal spikes in blood pressure. Approximately 12,000 new spinal cord injuries occur in the U.S. each year. A majority of injuries occur from motor vehicle accidents, falls, work-related accidents, sports injuries, and penetrations such as stab or gunshot wounds.

Our Membership Community

Our membership community provides a lifeline for many individuals that are focused on regaining their independence and improving their quality of life––whether they are leaving rehab after sustaining a spinal cord injury, learning to live with symptoms of a spinal cord disorder, or have spent years of frustration coping with disability. We provide members guidance and resources on a variety of topics they are passionate about, such as employment, affordable housing, transportation, health care, home- and community-based independent living, education, peer support, and leisure and recreation.
